Attached please find the revised Affidavit of Suitability for the Reverend Samuel West who will be visiting Chicago from November 7-13, 2023.  Fr. Sam will be staying at St. Josphat Church during his visit with the approval of Fr. Carlos Rodriguez and will be available to exercise priestly ministry as assigned.

The original revised letter has been mailed to the Archdiocese of Chicago.
